Have you ever noticed that sometimes when you or a loved one is released from the hospital, there isn’t very good communication between the hospital and your pharmacy in the community?

We are working closely with the Dauphin Regional Health Center to fix that communication gap. When you or your loved one is in the hospital, just tell them that the Dauphin Clinic Pharmacy will be looking after your discharge medications. Then tell the nursing staff to call us the day before your release, and we will make sure all the information from the hospital about your medications is collected. At your request (and at no additional charge) we will even deliver your medications to your bedside just before your release to save you a trip after discharge.
